Clinic Policies

CONSENT & INTAKE FORMS

All necessary intake and consent forms will be emailed to clients before their appointment. It is imperative these forms are filled out prior to your appointment. Please call us if you have questions or issues with any forms. 

CANCELLATIONS

Your scheduled appointment is reserved exclusively for you. Should you need to cancel or reschedule your appointment, please notify us 48 hours in advance to avoid a $50 fee for a late canceled appointment. All services re-scheduled or canceled on the same day of appointment or missed without notice will incur a $50 fee.

LATE ARRIVALS

Our scheduling is designed to permit the correct amount of time to complete your service. A late arrival will deprive you of precious treatment time. In fairness to others, your treatment must end on time so the other client's session can begin on time. If you are not able to be on time we will do our best to complete as much of your treatment as possible, however with some treatments it may be necessary to reschedule you. If you are 15+ minutes late for your scheduled appointment, you will be considered a "no-show" and charged a $50 fee. 

PAYMENT

We accept Cash, Debit Visa, MasterCard and American Express. We also have in-house and third party financing options available for several of treatments.

GRATUITIES

Gratuities are at your discretion and are not included in any service or package.

AGE 

All clients must be at least 18 years of age to receive any treatment at the clinic including injectables.

VALUABLES

We are not responsible for lost or stolen items and encourage you to leave valuables at home. 

PRICES & TIMES

All prices are subject to change without notice. All times stated are approximate.

Clean Slate Skin Bar and any of its employees reserve the right to refuse services at our discretion.

REFUNDS/ EXCHANGES 

Our return policy on product purchases is 30 days. The item must be unopened and in its original packaging.  Some exclusions may apply.

GIFT CARDS 

We offer online and physical gift cards. We do keep record, but please bring your physical gift card to your appointment with you.

Unfortunately, we do not offer refunds on gift cards, treatments, pre-purchased services or monthly promotions.
